1. Scope and Preparation
I discuss your service, previous CQC findings, known concerns and the areas where you need the most support. We then agree the scope of the review and request the most relevant evidence.

Where an on-site review is included, I assess the care environment, safety systems and day-to-day working practices. The areas examined will depend on the type of service and agreed scope.
5. Evidence Triangulation and Risk Assessment

Every review is tailored to your service, regulated activities, current risks and the reason you are seeking support. We use a structured, evidence-based approach informed by CQC’s current assessment approach, the five key questions and the regulations relevant to your service.
2. Evidence and Documentation Review
I review a proportionate sample of policies, audits, risk assessments, staff records, governance information and other compliance evidence. I consider whether documents are current, service-specific and being used in practice.
4. Staff and Leadership Discussions
I speak with staff and leaders to understand how procedures are applied in practice. This helps identify whether responsibilities are understood and staff can explain how they provide safe, person-centred care.
6. Findings and Improvement Plan
You receive clear feedback explaining what is working well, where further assurance is needed and what actions should be taken. Actions are prioritised so immediate risks can be addressed first.
