REQUIREMENTS
What CQC Needs to Add a Location
What to Send CQC
✓ Add a location: provider form – location details, regulated activities, dental chairs
✓ Updated Statement of Purpose – always required, or CQC returns the application
✓ Add a location: Manager application – or a full application if a new manager is needed
✓ Extra location section – one for each additional site
✓ How the site will be safe and well-led – a clear, site-specific answer
Your locations are a condition of your registration. To add one, you apply to vary that condition using CQC’s current Add a location forms, sent by email. CQC then assesses whether you can meet its requirements at the new site.
Have Ready for a Dental Site
✓ Infection prevention and control – decontamination room set up
✓ X-ray documents – critical examination, Local Rules, HSE registration
✓ Fire, Legionella, health and safety – risk assessments for the new site
✓ Medical emergencies – medicines and equipment in place
✓ Staffing and recruitment – safer recruitment records for new staff

AVOID DELAYS
Common Mistakes
Opening early
Treating patients at a site CQC has not added breaches a condition of registration, which is an offence.
Mismatched activities
Activities at the new site must already be in your registration, or you also apply to add them.
Old forms
CQC returns applications made on out-of-date forms.
Applying too early or not ready
CQC may return applications sent too far ahead, or for sites not ready to assess.
No updated Statement of Purpose
CQC returns the application without it.
Adding a Location FAQs
CQC Consultancy provides independent guidance. Only CQC decides whether to add a location. The provider and Registered Manager remain responsible for the accuracy of the application. Technical reports and certificates must be completed by competent professionals.
